I, the undersigned, grant The Creative Breakdown LLC and its clients, licensees, and assigns the irrevocable right and permission to record, photograph, film, and use my name, likeness, image, voice, and performance in connection with the above-referenced project. I grant the right to use this content in any and all media, now known or hereafter devised, worldwide, in perpetuity, for any lawful purpose including but not limited to advertising, promotion, broadcast, and digital distribution. I waive any right to inspect or approve the finished product and release all parties from any claims arising from such use.
